Arranging for a Proctor

Proctor Requirements

All exams must be taken in the presence of a designated and approved proctor. The following guidelines must be followed. All questions regarding proctors should be directed to the instructor for Math 141 or Math 142.

You are responsible for making arrangements for a proctor by contacting an official from one of the categories listed below. Those typically approved as proctors include:

• Officials at the ISU Outreach Center or Extension Office in your county
• Professional employees of accredited postsecondary degree-granting institutions, for example, registrars, deans, directors of college testing services or continuing education offices
• Elementary or secondary school superintendents, principals, teachers, or guidance counselors
• Professionals working for public or private libraries
• Commissioned officers of the military
• Clergy

No relative or immediate work supervisor may proctor an examination.

Additional Proctor Rules

1. The exams for the course are taken over the World Wide Web. Thus, you will need to use a computer at your proctor's place of business and under your proctor's supervision, with a web browser connected to the internet. The browser should be a fairly recent version (for details, see the online testing notes). Dates are somewhat flexible, according to the Detailed Schedule for Math 141 or Math 142.
2. You may not use your own computer or a computer in your own home for proctored exams.
3. Do not choose a proctor who uses AOL as their Internet Service Provider. AOL does not work with the online testing system.
4. Your proctor must have an email address.
5. You must use the same exam proctor for each of the exams in the course. When making arrangements with a proctor, make sure that they are able to supervise all the exams required for the course. If you cannot arrange this, you will have to obtain special permission from the instructor for Math 141 or Math 142 to use more than one proctor.
6. If you live in or near Ames, Iowa, you may take your proctored exams in the Mathematics Computer Lab (449 Carver Hall). Lab hours are posted on the lab web page.
   • If you wish to take exams in the lab, you will need to meet with one of the lab consultants in order to become acquainted with the lab testing rules and procedures. I suggest that you meet with the lab consultant before your first exam, but you may wait until the day of your first exam if you wish. Here is the contact information for the lab consultants:

   • When you meet with the lab consultant, you must then fill out the proctor form below with this lab consultant listed your official proctor. However, you may take exams in the lab during any of the open hours - whichever lab consultant that is on duty at that time will proctor your exam.
   • Occasionally other classes are scheduled in the lab. Be sure to check the posted schedule for room 449 on the day before you go to the lab to take an exam.

Online Proctor Registration

Once you have arranged to take the exams with a proctor, you and your proctor need to fill out and submit the online Proctor Form (see link below). Online proctor registration will be available beginning June 13.

1. First read the Instructions and Rules for Proctored Exams, and the Additional Proctor Rules above.
2. Meet with your proctor and fill out the online Proctor Form together. Follow the instructions on the form.
   • Proctor Form for Math 141
   • Proctor Form for Math 142
3. After you press the SUBMIT button on the form, a signature page will appear in your browser. You will need to print out this page, sign it and have your proctor sign it, and then mail or fax it to your instructor.
   • If for some reason the signature page does not appear, please contact your instructor for Math 141 or Math 142, and a new signature page will be sent to you via email.
4. Immediately after submitting the online Proctor Form, both you and your proctor will be contacted by email with instructions on the procedure for taking the online proctored exams, and you can then proceed to take exams at any time. If for some reason you do not receive these emails right away, please contact your instructor for Math 141 or Math 142.
