Arranging for a proctor
All exams must be taken in the presence of a designated
and approved proctor. The following guidelines must be followed. All questions regarding
proctors should be directed
to the instructor.
You are responsible for making arrangements for a proctor by
contacting an official from one of the categories listed below. Those typically approved
as proctors include:
- Officials at the ISU Outreach Center or Extension Office
in your county
- Professional employees of accredited postsecondary degree-granting
institutions, for example, registrars, deans, directors of college testing services
or continuing education offices
- Elementary or secondary school superintendents, principals,
teachers, or guidance counselors
- Professionals working for public or private libraries
- Commissioned officers of the military
- Clergy
No relative or immediate work supervisor may proctor an
examination.
Also, please note the following:
- The exams for the course are taken over the World Wide Web.
Thus, you will need to use a computer at your proctor's place of business and under
your proctor's supervision, with a web browser connected to the internet. The browser
should be a fairly recent version (for details, see the online testing notes). Dates are somewhat
flexible, according to the Detailed
Schedule for Math 141 or Math 142.
- You may not use your own computer or a computer in your own
home for proctored exams.
- Do not choose a proctor who uses AOL as their Internet Service
Provider. AOL does not work with the online testing system.
- Your proctor must have an email address.
- You must use the same exam proctor for each of the exams in
the course. When making arrangements with a proctor, make sure that they are able
to supervise all the exams required for the course. If you cannot arrange this, you
will have to obtain special permission from the instructor for using more than one
proctor.
- If you live in or near Ames, Iowa, you may take your proctored
exams in the Mathematics
Computer Lab (449 Carver Hall). Lab hours are Monday-Friday
1-5pm.
- If you wish to take exams in the lab, you will need to arrange
a meeting with one of the lab consultants, Ruchi Dhingra or Viveck Rao,
to become acquainted with the lab testing rules and procedures. In addition, you
must then fill out the proctor form below with Ms. Dhingra or Mr. Rao listed your
official proctor. Here is their contact information:
- Occasionally other classes are scheduled in the lab. Be sure
to check the posted schedule on the day before you
go to the lab to take an exam.
Once you have arranged to take the exams with a proctor, you
and your proctor need to fill out and submit the appropriate online proctor form:
Instructions are on the form. After you press the SUBMIT button
on the form, a signature page will appear in your browser. You will need to print
out this page, have your proctor sign it, and then mail or fax it to your instructor.
Immediately after submitting the online Proctor Form, both
you and the proctor will be contacted by email with instructions on the procedure
for taking the online proctored exams, and you can then proceed to take exams at
any time.
