Proctor Registration Form

Please enter student and proctor information below. Note that information must be provided in all fields, with the exception of the proctor fax number. Be careful to enter the information correctly, as you will not be given a second chance to correct it.

You should have been assigned a 9-digit student ID number by Iowa State University. Enter that number in the second field below. If you do not know your student ID number for some reason, use your Social Security number instead.

It is vital that the student and proctor email address are correct, as important instructions will be sent to these addresses. Make sure that you are careful with spelling and capitalization, and that you make the correct distinction between dashes (-) and underscores (_), the letter l ("ell") and the number 1, and the letter O ("oh") and the number 0.

On the other hand, if you want to correct information for a previously registered proctor, do not fill out this form. Instead, send an email to the instructor with the corrected information.
